About the In-Home Behavior Support Service Grant Program

 

 

This is a program funded through a Family Support Grant from OPWDD-Office for People with Developmental Disabilities (formally known as OMRDD) for individuals with Autism and Dual Diagnosis living at home with their families. Your behavior specialist will come to your home to work with you and your family member on how to deal with difficult behaviors at home.   

 

Visits are scheduled between you and your assigned Behavior Specialist.  

 

Your behavior specialist will develop an appropriate behavior plan with you for you to use at home to better manage difficult behaviors. Your behavior specialist will assist you with implementing this and will modify plans as needed. By working together you will:

 

Who is involved in the In-Home Behavior Support Service Grant Program?

To have success with this, we are a team. Full participation is required. By working together we will help you and your child make improvements at home. Everyone’s part is important.

 

Family members. You are a big part of this service. The more effort you put into this with your behavior specialist, the more results you will see. The Behavior Specialist will work directly with family members to give them the tools they need to make changes at home.

 

Key providers. Your behavior Specialist will maintain communication with your child’s providers as needed and/or requested. These may include physicians, psychologists, service coordinator, and any school officials that may help contribute to behavior plans.

What are the expectations for participants in the In-Home Behavior Support Service Grant Program?

Behavior specialists. Your behavior specialist will set up visits with you as needed and will call to remind you of these appointments. There may be times when a crisis situation with another family, bad weather or traffic will force us to reschedule a visit. Your behavior specialist will contact you directly if this occurs and will reschedule the visit as soon as possible.

 

Parent/Guardian-prior to beginning services, you will sign a contract with your behavior specialist, outlining goals and expectations. We understand that things do come up unexpectedly. It is important that in the event that you cannot be available for a scheduled visit, you call your behavior specialist to cancel at least 2 hours ahead of time. Understand that this service is in high demand and with frequent cancelations, you will be moved to the waiting list until your circumstances change and you can make appointments routinely.

 

As part of your behavior plan you will be keeping track of your child’s behaviors. In addition to this you will fill out a parenting-stress survey monthly to assess the level of behaviors that are presented at home and the way you are coping with stress associated with the behavioral difficulties at that time.

How long is the service?

This service is time limited. Initially visits will be weekly and then you and your behavior specialist will decide how often they are needed. The visits will gradually fade as you begin to utilize new approaches to improve behaviors at home. Typically the service is provided to you for 6 months depending on your need.

Are there any additional services?

If there are additional supports at home or at school, your behavior specialist can assist you in providing and making referrals to appropriate services.

 

If, once this service is completed and a few months go by, you feel you need more behavioral support at home, please feel free to contact us. We will assist you as much as we can until a spot opens up to resume visits.
